qualquer motivo, desejar alterar a ordem de prefer\u00eancia estabelecida acima poder\u00e1, em vida, indicar formalmente o benefici\u00e1rio de sua escolha para fins de recebimento do amparo social, mediante preenchimento de formul\u00e1rio pr\u00f3prio e apresenta\u00e7\u00e3o dos documentos exigidos.<\/p><p>Este receber\u00e1 o valor, visando cobrir as despesas com o
